To assess your eligibility for funding, you’ll need to provide details about your household size and income. If you qualify, you’ll also need your appointment date, the clinic’s name, how far along you are in the pregnancy (including the date of your last period), and the total cost quoted by the clinic. If you don’t have all this information, you can still call to start the screening process, but completing the funding might take multiple calls.
If you are eligible for funding, you will be informed of the amount available to you without needing a follow-up call. The NAF will notify the clinic of their contribution the day before your appointment, and the clinic will deduct this amount from your bill. If you need additional assistance, you will receive a Case Manager’s name and direct number for further support. Keep this information in case of any changes to your appointment or procedure cost.
